FOR HEALTHCARE PROFESSIONALS

Referrals & Professional Resources at EnCare MD

When a patient needs focused pain or infusion support, the next step should be easy to understand.

EnCare MD accepts referrals for Post-Acute Pain & Recovery, Interventional Pain Management, and Infusion Medicine at our Dallas practice near Oak Cliff and the Bishop Arts District. Physicians, surgeons, hospital discharge teams, care coordinators, and other healthcare professionals can refer patients directly. Patients may also contact EnCare MD themselves when a referral is not required for the service they need.

Three Referral Pathways

Our role is to understand the reason for referral, review the information that matters, and help the patient move into the appropriate next phase of care without unnecessary delays or confusion.

Post-Acute Pain & Recovery

Designed for medically stable patients who need focused pain support after surgery or another procedure, hospitalization, emergency treatment, or an acute injury. Care is time-limited and can continue for up to 90 days, although many patients need support for a shorter period.

This pathway can be especially useful when a patient is home and medically stable but still dealing with significant pain, medication questions, limited function, or a difficult transition before the next scheduled follow-up.

Same-day or next-business-day appointments may be available.

Interventional Pain Management

Refer patients who need a focused evaluation of spine, joint, nerve, musculoskeletal, or other pain concerns. Care is led by Dr. Nenna Nwazota, who is board-certified in Anesthesiology and Pain Medicine. Her approach is interventional and multimodal, with an emphasis on non-opioid treatment strategies.

The evaluation determines what makes sense next. That may include an image-guided procedure, medication strategy, infusion therapy, additional testing, conservative treatment, or coordination with another clinician.

Infusion Medicine

EnCare MD provides appointment-based outpatient infusion therapy for patients with established treatment plans. Services include selected specialty medications, IV iron, osteoporosis therapies, IV antibiotics, medically indicated hydration, migraine rescue protocols, sickle cell hydration and pain support, and other supported infusion or injection therapies.

Before the first treatment is scheduled, the team reviews the order and any required records, lab results, authorization, or medication-specific information.

What to Include With a Referral

A well-documented referral helps EnCare MD understand the clinical question and can reduce unnecessary back-and-forth for both the patient and the referring team. When relevant and available, please include:

  1. Discharge summary or operative note
  2. Current medication list and recent opioid exposure
  3. Relevant imaging and laboratory results
  4. Known allergies
  5. Pertinent medical and substance-use history
  6. Expected recovery course or activity restrictions
  7. Referring clinician contact information
  8. The specific clinical question or goal of the consultation

Not every referral requires every item. The most useful records are the ones that explain what happened, what has already been tried, and what you would like EnCare MD to evaluate or help manage.

What Referring Clinicians Can Expect

EnCare MD provides focused pain and infusion support while keeping the patient’s broader care connected. Depending on the referral, that can include:

  • Review of relevant records
  • Focused pain or infusion evaluation
  • Image-guided interventional treatment when appropriate
  • Multimodal pain-management recommendations
  • Short-term follow-up during post-acute recovery
  • Coordination of infusion requirements
  • Communication with the referring or treating clinician when appropriate and authorized
  • Transition back to the treating team or another appropriate care pathway when EnCare MD’s role is complete

For Post-Acute Pain & Recovery, the surgeon or other treating clinician continues to manage the underlying surgical, medical, or injury-related condition while EnCare MD focuses on pain-related needs during recovery. For Infusion Medicine, the clinician managing the underlying condition generally continues to direct the medication plan while EnCare MD provides and monitors the infusion.

Keeping Care Connected

The clearest referral relationships are the ones in which each clinician’s role is understood. EnCare MD is designed to provide focused support without disrupting an established treatment plan. Surgeons continue surgical follow-up. Specialists continue management of the underlying condition. EnCare MD contributes pain-management expertise, short-term recovery support, interventional treatment, or outpatient infusion therapy where appropriate.

If a patient develops a complication, medical instability, or another problem that falls outside the reason for referral, the priority is getting that patient back to the appropriate treating clinician or level of care.

When Patients Can Contact EnCare MD Directly

Patients can contact EnCare MD directly for Interventional Pain Management and Post-Acute Pain & Recovery. Insurance requirements, certain procedures, or specific treatments may still require a referral or prior authorization.

Infusion Medicine requires appropriate clinical review and the treatment-specific order, documentation, labs, authorization, or other information needed for the therapy being requested. If a patient is unsure which service fits, EnCare MD can help determine the appropriate next step.

Getting a Referral Started

Start by identifying the service that best matches the patient’s current need.

The EnCare MD team can then determine what records or treatment information are needed to review the referral.

An appointment does not guarantee that an existing controlled-medication regimen will be continued or that a particular medication will be prescribed. Medication decisions are based on the current clinical evaluation.

Yes, in many cases. The fit depends on why you are seeking care, what treatment is already in place, and what type of additional evaluation or support is being requested.

Questions Referring Professionals May Have

Consider referral when a medically stable patient needs focused pain support after surgery, hospitalization, emergency treatment, or an acute injury and the need falls within EnCare MD’s scope.
No. EnCare MD provides focused pain or infusion support. The surgeon, specialist, or other treating clinician continues to manage the underlying condition and the aspects of care specific to it.
Relevant discharge or operative documentation, current medications and recent opioid exposure, imaging or labs, allergies, pertinent history, recovery restrictions, referring-clinician contact information, and the specific reason for referral are especially useful.
When appropriate and authorized, EnCare MD can coordinate with the referring or treating clinician regarding the focused care being provided.
The patient can transition back fully to the treating team or, when clinically appropriate, continue through another EnCare MD care pathway. Post-Acute Pain & Recovery is intentionally short-term rather than open-ended.
